Factory Repair Order Form
Our Repair Department stands ready to serve you in any way possible. Some of those ways include warranty repair or replacement, or refurbishing/rebuilding any product we manufacture. If you have an Elkhart Brass product that needs our attention, please email us at repair@elkhartbrass.com or call us at 800-346-0250 (ext. 269) to request an RGA (Returned Goods Authorization) number for any and all products being returned, whether for warranty consideration, refurbishment or rebuild.
Then simply fill out and print the form below.
Please be sure to enter the RGA number on this form; tell us which product (or products) you are sending; give us an explanation of the problem with the product (or products) you are sending; and inquire about the warranty status of the product or notify us that you need an estimate before we proceed with the repair. Most repairs are turned around in 1-2 weeks after receipt of product and authorization to proceed.
